Job description

Reporting to the Assistant Director of Care, the Health Informatics Nurse’s main function will be to oversee and ensure compliance with all aspects of the RAI and care planning processes and reporting requirements.

Duties
  • To oversee the RAI process in accordance with the home policies, MOHLTC regulations, College of Nurses Standards of Practice and inter-RAI requirements;
  • Collaborate with the software provider to trouble shoot and maintain the software system;
  • Monitor and provide on-site support to all RAI users in the facility;
  • Provide ongoing education and computer training;
  • Working with RAI users to develop processes that ensure data accuracy, efficient data collection process assessment completion and data utilization;
  • Generate RAI MDS reports and facilitating their use in clinical and operational decision-making;
  • To oversee the Care Plan process in accordance with the home policies and MOHLTC regulations;
  • Lead monthly care plan meetings;
  • May be required to work 12-hour Charge RN shifts;
  • Support resident safety initiatives, identifying and addressing clinical risks to ensure high-quality, safe, and person-centered care;
  • Other duties as assigned.
Qualifications
  • Registered Nurse;
  • Current College of Nurses of Ontario Certificate of Competence;
  • Current MDS Credentialing Certificate;
  • Excellent understanding of the relationship between the RAI process, the care plan, and quality resident care;
  • Excellent understanding of the development of the RUG-III classification system;
  • Excellent oral and written communication skills;
  • Demonstrated organizational, time management and leaderships skills;
  • Ability to work with and relate to interdisciplinary team members;
  • Excellent working knowledge of various computer software;
  • Current clear Criminal Record Check and Vulnerable Sector Screen required;
  • Current WHMIS certificate required;
  • Ability to adhere to all infection prevention and control protocols and screening requirements;
  • Must comply with COVID-19 immunization directives as per the Ministry of Long-Term Care.
