CONTRACT SUMMARY: Reporting to the Take Heart Steering Committee and the Facility Liaison, assists in the planning, development, coordination, implementation, and ongoing evaluation of safe, community-medical based cardiac rehabilitation exercise programs. Assists in the coordination of client care through the recovery continuum from acute care to the community.
TYPICAL DUTIES AND RESPONSIBLITIES:
- Ensures all clients receive medical approval before commencing the program.
- Monitors the exercise environment for client safety and tailors exercise to the needs and levels of the clients.
- Adheres to best practice guidelines, as established by the Take Heart Steering Committee.
- Assesses client and incorporates physician’s interpretation and evaluation of clinical stress and metabolic test data and cardiac investigation, e.g. MIBI, ECHO, etc.
- Applies principles of exercise physiology and kinesiology to individual participants. Provides safe, individual exercise prescription, such as aerobic, strength and flexibility exercises, and establishes progression of exercise training based on initial and ongoing client assessment.
- Designs, conducts and supervises onsite exercise sessions. Monitors clients’ response to exercise. Responds to complications and modifies exercise to address limitations.
- Educates clients and their families by conducting one-on-one interviews. Assists program participants in recognizing the signs and symptoms of cardiovascular and/or respiratory distress.
- Assess presence and extent of modifiable cardiovascular risk factors.
- Coaches client and/or family to identify, develop and maintain health promoting behaviours within the context of the illness or disease. Provides motivational support to client and family regarding long-term behavioural modifications and exercise.
- Provides ongoing psychosocial support in exercise class and with follow-up.
- Reviews medications and monitors compliance with exercise prescription.
- Functions as an educational resource for clients, families, and other members of the client care team.
- Provides leadership in urgent and emergency client exercise related situations.
- Maintains clear and concise client records and statistics.
- Provides interpretation/application of results of exercise stress testing to clients and other health professionals, as need be.
- Provides physician with client’s progress report within one month of program completion.
- Contacts the client’s physician if any concerns arise regarding the client’s health or their progress in the program.
- Assists to recruit, orient, and/or train volunteers, students, and other Program Deliverers in the Take Heart Program.
- Provides leadership, supervision, and support for further development for volunteers and students working with the Take Heart Program.
- Participates in monthly meetings with the Take Heart Steering Committee.
- Performs other related duties as required.
CONTRACT SPECIFICATIONS:
A. Education, Training and Experience
- Current First Aid, CPR-C & AED certification
- Exercise related science degree or medical related degree or equivalent
- Recognized exercise certification; BCRPA strength training program or equivalent (or able to obtain within 6 months)
- Cardiac rehabilitation exercise experience within the last 3 years
- ACSM Exercise Specialist, CSEP Exercise Physiologist (or able to obtain within 6 months) or Registered Nursing Degree with exercise experience
- Possess current registration with a professional association (e.g., RNABC, BCAK, CSEP, ACSM)
- Acceptable Police Information Check with Vulnerable Sector Screening
- Possess professional liability coverage or a minimum of $5 million dollars, naming the program’s organization as an insured party
- Cardiac care specialty course (preferred)
- Attend a VIHA recognized course on recognizing & dealing with crisis or acute medical incidents (preferred)

- Ability to communicate effectively, both verbally and in writing, with clients, families, team members and the community
- Ability to teach and lead group sessions
- Ability to demonstrate knowledge and utilization of adult education principles and motivational techniques
- Ability to coach and counsel individuals and families
- Ability to assess cardiovascular and pulmonary status and symptoms of heart failure, cardiovascular, or pulmonary distress
- Ability to work within the prescribed policies, guidelines, and directives
- Ability to physically carry out the duties of the position
Program Days: Mondays and Wednesdays
Program Times: 1:15 – 6:15pm (class times: 1:15-2:45pm, 3:00-4:30pm, 4:45-6:15pm)
Note: Program Administration Time, Assessments, and Orientations fall outside of program hours. Total hours fluctuate between 11 – 16 hours most weeks.
This is a contract position. Contractors are expected to have First Aid & CPR, current certifications, clear criminal record check, and 5 million liability insurance.
