A regional health service in Owen Sound is looking for a part-time Social Worker to join their Diabetes Education department. The role involves assessing and supporting children and families at risk of diabetes. Candidates must possess a Bachelor’s degree in Social Work and current registration with the Ontario College of Social Workers. Strong leadership skills and experience in children's health are essential. This position demands flexibility and the ability to work all shifts, including weekends, contributing to a collaborative health care environment.
Reporting to the Manager, Diabetes Education, and the Social Worker for the Pediatric Diabetes Program is responsible for assessing, triaging, counseling and referring children and families who have or are at risk of diabetes. Client/family contact may be initiated when the child is a hospital in-patient or in the ambulator program. The Social Worker works collaboratively with other members of the Diabetes team to understand the complex interaction between the child/family and his/her environment, and assess for risk using bio-psycho-social assessment and intervention strategies to support families, and promote self–management/coping while living with a chronic disease.
