POSITION LOCATION: OWEN SOUND
DEPARTMENT: DIABETES EDUCATION
UNION AFFILIATION: OPSEU – HPD
STATUS: PART-TIME
WAGE RATE: $39.16 - $50.77

Reporting to the Manager, Diabetes Education, and the Social Worker for the Pediatric Diabetes Program is responsible for assessing, triaging, counseling and referring children and families who have or are at risk of diabetes. Client/family contact may be initiated when the child is a hospital in-patient or in the ambulator program. The Social Worker works collaboratively with other members of the Diabetes team to understand the complex interaction between the child/family and his/her environment, and assess for risk using bio-psycho-social assessment and intervention strategies to support families, and promote self–management/coping while living with a chronic disease.

Qualifications / Skills / Abilities:
  • Bachelor’s Degree in Social Work
  • Current registration with the Ontario College of Social Workers and Social Service workers
  • Recent (within the last 2 years) relevant Social Work experience in the area of children’s health and/or diabetes
  • Self-directed with ability to work independently with minimal supervision
  • Demonstrated management of the multiple priorities and demands of dynamic work environments and the need to work flexibly within limited time resources
  • Demonstrated abilities to assess children and families for the need for service; formulate and implement an intervention plan with child/family members, significant others and multidisciplinary team
  • Experience working with issues related to high-risk family support
  • Demonstrated knowledge of external resources and a commitment to work collaboratively with other community agencies to establish effective patient care plans
  • Demonstrated leadership, problem solving, and decision-making skills to provide advocacy when needed
  • Demonstrated commitment to continuous learning
  • Ability to utilize Microsoft Outlook, and Word
  • Ability to work all shifts, including weekends; ability for multi-site coverage when required
  • Recent satisfactory performance and attendance record
