The Community Crisis Centre is a client-centered mental health service delivery model that provides crisis response services to individuals who are experiencing an acute mental illness and/or psychosocial crisis. Using the principles of client-centered care, the Social Worker will provide services to clients, which includes assessment, and planning, crisis support/counseling, medical intervention, environmental interventions and crisis stabilizations, provide appropriate referrals to ongoing services and supports, monitor and evaluate the achievement of goals, provide information, consultation/collaboration, and advocacy. This position contributes to the hospital mission by being a team member, which ensures confidentiality, quality, and client-centered health care in the community.
