As a Care Designer, you are responsible for the day-to-day operations related to driving business results through client experience and caregiver management. You will drive growth through the following main duties and responsibilities:

Consult Conversion:

  • Conduct Caring Consults, assess clients regarding physical, emotional, and environmental needs, and advise on client care; achieve a conversion goal of 90%.
  • Drive business interest by engaging with community leaders and professional organizations to establish long-term relationships that generate word-of-mouth and professional referrals.
  • Develop, promote, and ensure high-quality, innovative customer service.
  • Act as an advisor during Caring Consults to ensure clients and contacts have all necessary information.

Client Care Delivery and Experience:

  • Attend all first visits with caregivers to ensure client needs and care plans are executed as per the Service Agreement; set and role model expectations.
  • Conduct supervisory visits to ensure caregiver fit and quality of care, delivering a premium client experience impacting Net Promoter Scores of 9 or higher.
  • Develop individualized care plans and update as needed.
  • Educate clients on self-care and evaluate responses to care provided.
  • Monitor risk issues, analyze trends, and adapt care accordingly.
  • Coordinate care, implement clinical solutions, and evaluate client outcomes based on goals.
  • Lead caregiving teams by setting goals, assigning tasks, coaching, and providing ongoing education.
  • Lead the client experience to ensure caregivers do whatever it takes to bring peace of mind to clients and their families.

Staff Management:

  • Responsible for staff hiring, training, and performance management to meet business needs and ensure compliance.
  • Manage education programs to support continuous learning, including Discovery Day and monthly in-service programs.
  • Conduct first and supervisory visits to evaluate caregiver performance and satisfaction, impacting employee experience and Net Promoter Scores of 9 or higher.
  • Liaise with scheduling to communicate client requirements and caregiver assignments.
  • Ensure client coverage, especially in urgent cases, cancellations, and limited availability; assist with scheduling as needed.
  • Maintain AMS (Attendance Monitoring Investigations) percentages below 5% and manage caregiver performance.

Administration:

  • Maintain accurate documentation, including care plans, employee files, and client records.
  • Review and update records quarterly to ensure 100% accuracy.
  • Ensure compliance with health, employment, and safety legislation.
  • Approve visits and premiums for payroll and billing cycles.

Availability:

  • Flexibility required (availability 7 days/week).
  • Schedule provided 2 months in advance.
  • Part-time role, approximately 20 hours/week (nights and weekends).
  • On-call evenings, holidays, and weekends.

Business Relationships:

  • Report to the Franchise Partner.
  • Lead the caregiving and Care Ambassador teams.
  • Work closely with the Care Services Center.

Qualifications and Requirements:

  • Registered Practical Nurse or Registered Nurse licensed in Ontario.
  • CPR and First Aid Certification.
  • Valid driver’s license and access to a car.

Experience:

  • Strong computer skills, proficient in G-Suite and electronic medical documentation (experience with Alayacare is an asset).
  • Excellent written and verbal communication skills in English.
  • Organized, with strong planning, time management, and multitasking skills.
  • Experience in goal setting and achievement.
  • Customer service and leadership experience are assets.
  • Background check with Vulnerable Sector Search (updated every 5 years).
  • Negative TB test (updated every 5 years).
