Salary Range: $61,368 - $76,710 per annum, plus comprehensive benefits

Hiring Range: $61,368 - $69,039 per annum, plus comprehensive benefits

Work mode: Onsite

Location: This position is a mobile position working on a team that supports 23 Long Term Care homes across the Central West region. This includes Dufferin, Caledon, Brampton, Malton and parts of Etobicoke

Hours of work: 35 hours per week

Who we are: Behavioural Supports Ontario (BSO) Provides behavioural health care services for older adults in Ontario with, or at risk of, responsive behaviours/personal expressions associated with dementia, complex mental health, substance use and/or other neurological conditions. All behaviours have meaning and personal expressions are often an expression of an unmet need.

Staff are to have emotional intelligence ability to deliver person-centered care in an authentic way that is meaningful to the resident and their family.

POSITION SUMMARY

The Resident Care Assistant works within the Behavioural Supports Ontario (BSO) framework to support Long-Term Care in-house teams across the Central West geography. The Assistant provides guidance for and assistance with personal care and engagement strategies to support residents with responsive behaviours and personal expressions.

The incumbent reports to the Manager of Practice, Innovation and Education (PIE) and works with an interdisciplinary team that provides timely assessments and interventions for individual residents and supports transitional management between health care providers. The Resident Care Assistant assists with enhancing the quality of life for residents with responsive behaviors and personal expressions in long term care sites across the Central West region as part of the Central West BSO LTC Mobile support team.

MAJOR RESPONSIBILITIES
  • Adheres to BSO principles and standards of practice when providing care and communicating with families and service providers
  • Works within their scope of practice and under the clinical leadership of the Clinical Behavioural Support practitioner on the CW BSO LTC Mobile team
  • Works in collaboration with other members of the BSO LTC Mobile Support team (Clinical Behavioural Support, Neurobehavioural Nurse Practitioner, Psychogeriatric Resource Consultant (PRC), Therapeutic Recreation Consultant) to support the embedded BSO Leads in assigned LTC homes.
  • Reviews and supports development of the care plan, along with the interdisciplinary team members
  • Works alongside the interdisciplinary team during the provision of care and activities of daily living to guide, model and coach effective strategies specific to the resident’s needs.
  • Assist the BSO team to plan, organize and facilitate activities of daily living and programming for residents in the prevention and management of responsive behaviours, utilizing tailored strategies with specialized training received
  • Supports and demonstrates Montessori© or behaviour reduction activities in collaboration with the LTCH staff
  • Engages with residents to determining triggers and mitigate escalating responsive behaviours through appropriate care techniques.
  • Appropriately documents observations “before, during, and after” behaviours observed, interventions trialled and their outcomes, following documentation policies and procedures
  • Informs the care team of any changes or concerns identified in a timely manner
  • Respects residents’ individuality as a person, including cultural, religious and identity diversity
  • Participates in ongoing education and training within their scope of practice related to best practices and responsive behaviours (e.g. GPA, U-First)
  • Participates in continuous quality improvement through teaching, modelling and trialing new interventions related to personal expressions (to support continuous learning and integration of best practices to support residents)
  • Shares information in a safe, ethical, and respectful manner based on the needs of the resident. Exercises considerable judgment, tact and discretion in preparing, disclosing and handling information of a confidential and/or sensitive nature.
  • As a part of the team, support resident transitions into the LTCH from acute and/or tertiary care and community settings.
  • Participates in responsive behaviour meetings, education and/or training as required by the BSO Network and Peel LTC
  • Completes other duties as assigned
  • Carries out all duties with appropriate attention for resident safety.
QUALIFICATIONS
  • Minimum of three years work experience providing direct caregiving for residents in a Long-Term Care home
  • Knowledge and experience with the application of various interventions to support individuals with responsive behaviours (personal expressions) associated with dementia, complex mental health, substance use and/or other neurological conditions under direction and collaboration of BSO clinical providers
  • Experience with the implementation of behaviour management strategies (Montessori© techniques, GPA, U-First etc.) to support resident care plan
  • Education at minimum, must meet the personal support worker program requirements set by the Ministry of Colleges and Universities for an Ontario post-secondary institute or a diploma in nursing (with good standing).
  • Must have proof of completion of a personal support worker program or higher-level related education with proof of graduation provided by the education provider
  • Training in U-First, GPA, DementiAbility, GPA Coach, and Montessori© based programming are considered an asset.
  • Strong communication skills and the ability to work effectively within an interdisciplinary team environment to carry out assigned tasks
  • Ability to communicate effectively, using person-centered language, with residents and families
  • Initiative to seek advice and direction from regulated health professionals as needed
  • Self-motivated and able to act on own initiative, within the scope of the position’s responsibilities, using good judgment and discretion
  • Must have effective oral and written communication skills in English
  • Change Management knowledge, skills and training as asset.
  • Strong organizational and time management skills
  • Ability to travel to the various Long Term Care centres/homes across the Central West geography
  • Valid “G” licence in good standing and access to a vehicle

Work Mode & Job Location

On site: This position is a mobile position working on a team that will be supporting 23 LTC's across the Central West region. This includes Dufferin, Caledon, Brampton, Malton and parts of Etobicoke. The frequency of on-site activities may vary and is based on operational requirements.

Candidates must be available to flex work hours based on resident need, with start times as early as 7AM and end times as laste as 8PM.
