Overview
Registered Practical Nurse / Registered Nurse – Temporary Full-Time (37.5 hrs. / week) with Access Clinic for Primary Care. The clinic, established through the Brantford Brant Norfolk Ontario Health Team, provides primary care services for patients without a primary care provider, those transitioning to another provider, or those whose providers are outside of the BBN community. Implementation is supported by the Norfolk Family Health Team, Primacare Community Family Health Team, and the Grand River Community Health Centre. These organizations deliver quality health care and transformative support to individuals who face social, economic, and other systemic barriers, through primary health and chronic disease management, health promotion and education, outreach and social supports, and mental health services.
Patients access care from a multidisciplinary team including family physicians, nurse practitioners, registered nurses, social workers, medical office assistants, a registered dietitian, occupational therapist, kinesiologist, and pharmacist. The clinics also support the broader community with services to enhance health and wellness of individuals and families.
General Overview
The Registered Nurse / Registered Practical Nurse (RN / RPN) will work as part of the Access Clinic team and interprofessional primary care team. The RN / RPN will complete patient intake and initial assessments, support care planning, liaise with referring physicians and providers, and ensure continuity of care across transitions. The position includes contributing to the collection and submission of site-specific data and supporting program evaluation.
This role will be based at the Simcoe and Paris sites, with availability required for days and some evenings.

Responsibilities
Registered Nurse (RN / RPN):
- Receive patient referrals, complete the initial intake and assessment, and support the development of the patient plan of care.
- Facilitate system navigation and liaise with referring physicians and community health and social service agencies to ensure effective and timely communication and continuity of patient care.
- Manage site-specific data entry and evaluation activities.
- Provide complete patient assessment, including vital signs and pre-examination procedures. Gather and assess objective and subjective data from the patient.
- Document data and assessments in the patient record (EMR) and related systems to ensure standard documentation and coding.
- Assist providers in all aspects of consultation, treatment, procedures, and follow-up care.
- Administer medical regimens and nursing interventions as appropriate for the setting and competency.
- Perform medication reconciliation, support patient compliance and education.
- Operate medical equipment.
- Provide health education and health promotion responsive to client needs.
- Triages calls for urgent appointments, referrals for patient consults and testing, and related documentation.
- Maintain general office skills including keyboarding, software use, filing, and communication tasks.
- Administers and records immunizations with patient education on risks and benefits.
- Lead chronic disease prevention, management and self-care; lead community analysis and planning; actively lead quality improvement initiatives.
- Maintain clean, orderly, and well-stocked examination rooms and nursing station; sterilize equipment including IPAC procedures.
- Identify, plan, and prioritize nursing interventions; develop and refine care plans with the team.
- Provide in-person and telephone counseling, triage, and follow-up; act as an evidence-based champion in delivering care.
- Offer patient health counseling and instruction, including with families and caregivers.
- Report to the executive director or delegate and work closely with other members of the interprofessional primary care team.
- Participate in quality improvement initiatives and attend relevant planning meetings to support clinical flow.
NOTE: The full-time RN/RPN will be the first point of contact for the Program and will have additional triage responsibilities, as well as responsibilities related to program evaluation and data coordination/management.
Requirements
- Member in good standing with the College of Nurses of Ontario
- Current membership with the RNAO / RPNAO (LAP Insurance)
- Current CPR for Health Care Providers certification
- Working knowledge of acts, legislation, and professional standards governing nursing practice (PHIPA, RHPA, CNO Standards & Guidelines)
- Nursing care knowledge related to primary care, community health, and chronic disease management
- Understanding of social determinants of health and their impact on newcomer and migrant communities, homeless or underhoused individuals, and 2SLGBTQ communities
- Experience/interest in working with communities facing barriers to health care due to poverty, social isolation, homelessness, homophobia/transphobia, racism, classism, mental health, and/or substance use challenges
- Demonstrated ability to work effectively within a multidisciplinary team
- Experience and comfort with an electronic medical record (EMR)
- Working knowledge of Microsoft Office (Word, PowerPoint, Teams) is an asset
- Experience with health program planning, implementation, and evaluation is an asset
