What will you do?

· Respond to patient referrals from Care Coordinators to properlyadminister physiotherapy services .

· Assesspatient's physical abilities to determine the impact of the patient’s conditionon their ability to function in their environment.

· Analyze, interpret, develop, and implement treatment programsbased on assessment results.

· Oversee patients progress and adjust treatment programs whenneeded, to ensure therapy goals are met.

· Assign physiotherapy programming to a rehabilitation assistantbased on patient needs and supervise according to professional standards.

· Educate and train patients, their families, caregivers, and/orSDMs, on how to progress with the treatment plan.

· Link patients with other in-house services or outsideorganizations if needed.

· Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected.

· Accurately document and chart patients’ assessment results, goals,treatment plans, and interventions.

What must youhave?

  • RegisteredPhysiotherapist in good standing with the College of Physiotherapists ofOntario.
  • In-depthknowledge of physiotherapy services and other community resources.
  • Goodunderstanding of the roles of health care professionals.
  • Relevantphysiotherapy experience in a community/health care environment.
  • Comprehensiveand up-to-date knowledge of physiotherapy tools, processes, equipment, andassistive technology.
  • Knowledgeof funding agencies related to the physiotherapy recommendations.
  • Practicalknowledge and understanding of relevant legislation (e.g. regarding theprovision of health care services, privacy, health, and safety, etc.).
  • Comprehensiveknowledge of the standards of practice and professional guidelines set forthby CPO.
  • Abilityto prioritize professional duties, manage multiple patients, andefficiently organize workload.
  • Stronginterpersonal and collaboration skills to work with diverse patientgroups, Care Coordinators, family members/caregivers/Substitute DecisionMakers, health care professionals, community organizations and serviceproviders.
  • Effectivelistening, observation, and facilitation skills.
  • Abilityto communicate information effectively through a variety of meansincluding reports, letters, meetings, and presentations.
  • Workingknowledge of computer software (email, internet) and Microsoft Office

applications (Teams, Word, Excel).

  • Musthave valid driver’s license and access to a vehicle.
  • Advancedoral and written proficiency in English is essential.
  • We have amandatory COVID-19 vaccination policy. As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination statusprior to start date.
