Qualifications

  • Successful completion of internal legislated and mandated training is required.
  • Valid driver's license is an asset.

Responsibilities

  • Identifying program needs of individuals supported and assisting in the development and implementation of life plans.
  • Providing case management including personal care, medication administration, and crisis response.
  • Coordinating care with families, medical professionals, and service providers.
  • Providing life-skills training and emotional support; referring to professionals when necessary.
  • Maintaining documentation and monitoring finances for individuals supported.
  • Providing direction to part-time staff as required.
