Position Summary:

The Nurse Practitioner (NP) plays a critical role in providing comprehensive primary healthcare services, with a specialized focus on refugee health and pediatric care. The Nurse Practitioner will maintain and manage their own roster of patients, working within a multidisciplinary team to deliver high‑quality, client‑centered care. This role requires a commitment to providing accessible healthcare services and working collaboratively to improve health outcomes for vulnerable populations.

Main Responsibilities/Duties:
  • Provide comprehensive primary healthcare services to individuals and families, with a focus on refugee and pediatric populations.
  • Conduct health assessments, diagnose conditions, and develop treatment plans in accordance with best practices and clinical guidelines.
  • Prescribe medications and therapeutic interventions within the NP scope of practice.
  • Manage and coordinate care for patients with chronic and complex health conditions.
  • Collaborate with an inter‑professional healthcare team, including physicians, nurses, social workers, and community health professionals.
  • Provide patient education, counseling, and advocacy to promote health and well‑being.
  • Support the development and implementation of health programs tailored to the needs of refugee and pediatric patients.
  • Maintain accurate and timely electronic medical records (EMR) and documentation.
  • Participate in quality improvement initiatives and professional development activities.
  • Support and contribute to our company’s commitment to workplace equity, diversity, and inclusion.
Education and Employment Requirements:
  • Successful completion of a post‑graduate Primary Care Nurse Practitioner program.
  • Current Extended Class Registration with the College of Nurses of Ontario.
  • Proof of current membership or alternative professional liability coverage.
  • Current CPR certification (Level C or HCP).
  • Demonstrated experience in community and/or primary care.
  • Experience in chronic disease management and prevention.
  • Strong ability to work both independently and collaboratively within a healthcare team.
  • Excellent oral and written communication skills in a clinical setting.
  • Proficiency in the use of electronic medical records (EMR); Accuro experience is an asset.
  • Passion and experience in Refugee Health and Pediatric Care are considered strong assets.
Compensation & Benefits:
  • $52.10 - $68.41 per hour + on‑call stipend
  • Full‑Time (37.5 hours per week) Part‑time arrangements will be considered.
