Position : Nurse Practitioner (NP)

Employment : Permanent, full-time or part-time; salaried

Remuneration : Starting from $141,494 – $160,312 plus comprehensive health benefits. Relocation assistance available.

Start date : Flexible

Location : Vancouver, Downtown Eastside (DTES); Hope to Health Clinic is part of the Primary Care Networks (PCNs) initiative in Vancouver.

Overview

Hope to Health Clinic is looking to hire a Nurse Practitioner. The start day is flexible and the clinic will accept nurse practitioners whenever they are ready to start. The NP will work within a team-based approach to full-service family medicine and establish their own patient panel as required by the Ministry. The position is part of the PCNs initiative and requires willingness to work as part of a team to support PCN development in the community.

Responsibilities
  • Work collaboratively within a multidisciplinary care team that includes family physicians, an RN, LPN, social worker, peer support worker, and MOAs; in-house access to PT, OT, dietitian, psychologist, and psychiatry consulting services.
  • See about 8 to 10 patients per day in a 0.3 to 0.5 full-time equivalent role with flexible scheduling.
  • Contribute to a team-based, primary care model that serves complex clients with co-morbidities, including mental health, addiction, and psychosocial issues.
  • Engage in training and quality improvement (QI) activities; allocate protected time for indirect care and professional development.
  • Support the development of PCNs within the community and participate in ongoing clinical programs and research initiatives as applicable.
  • Relocation assistance available if required.
