The Acute Care Medicine Unit for Older Adults is an expanded service of Geriatric Medicine on 9C / D. The care model meets the growing needs of care for the older adult population with acute illnesses.

You will play a vital role as part of a multidisciplinary team that assists older adults with their acute health care needs, including providing care to those who have experienced significant functional decline in a short time frame, or who are experiencing multiple complex medical and / or psychiatric problems. In some situations, they may require rehabilitation / reactivation to return home.

In addition to your clinical responsibilities, you will provide professional guidance to other healthcare professionals, working autonomously and collaboratively within an interdisciplinary healthcare team. Your role will involve making referrals to specialist physicians and other resources as necessary. You will collaborate with patients, families, residents, and healthcare professionals to identify trends and patterns that impact the community's health and participate in evidence-based strategies to improve healthcare services. Your involvement in policy-making activities will also influence healthcare practices.

Qualifications / Skills and Education

About you :

  • Current registration as a Nurse Practitioner with the British Columbia College of Nursing Professionals.
  • Successful completion or awaiting results of the Objective Structured Clinical Examination (OSCE).
  • Completion of the Basic Cardiac Life Support Course, Level C.

Duties and Responsibilities

Diagnoses and treats previously undiagnosed patients / clients / residents for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner's scope of practice; writes orders for treatment and medications; provides first line care in emergencies and continuous care.

Monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment.

Establishes priorities for management of health, diseases, disorders, and conditions; provides follow-up treatment; communicates with patients / clients / residents and families about health findings, diagnoses and priorities, outcomes, and prognoses; supports and counsels patients / clients / residents in their responses to diseases, disorders, and conditions.

Collaborates and consults with medical staff colleagues: MDs and NPs, the healthcare team, and social service providers to determine the appropriate resources necessary for patient status. Develops and implements treatment plans. May admit and discharge patients / clients / residents to facilities according to medical staff bylaws.

Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable PHC medical staff rules / credentialing / privileging.

Provides mentorship and informal education related to patient care to registered nurses—specifically junior RNs, LPNs.

Participates in research and quality improvement initiatives contributing to patient care, access, flow, organizational quality, and medical staff quality.

Maintains a population health focus by implementing screening and health promotion activities for at-risk populations, patients, and families.

Participates in the education of nurse practitioner students and other healthcare professionals through preceptorship, role modeling, and knowledge exchange in clinical, classroom, and community environments to foster healthcare partnerships.

Develops, implements, and evaluates policies and procedures related to medical staff quality, organizational development, advanced practice nursing, nursing, interdisciplinary care, and health system practices.

Completes Encounter Records for evaluating Nurse Practitioner implementation as required by the Ministry of Health.

As per current Public Health Orders (Long Term Care / Seniors Assisted Living Provincial Health Order and the Health Sector Order), as of October 26, 2021, all employees working for Providence Health Care must be fully vaccinated against COVID-19. Proof of vaccination will be required.
