Job Summary
We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will have experience in providing care and support to individuals in various settings, including group homes, assisted living facilities, and home care environments. This role involves assisting clients with daily living activities, monitoring their health and well-being, and ensuring a safe and supportive environment.
Responsibilities
- Provide direct support to clients with developmental disabilities, Alzheimer's, dementia, and other memory-related conditions.
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation.
- Monitor patients\' vital signs and overall health status, reporting any changes to the appropriate healthcare professionals.
- Maintain cleanliness in living areas and assist with light housekeeping tasks.
- Prepare nutritious meals according to dietary needs and preferences.
- Administer medications as prescribed and ensure adherence to care plans.
- Utilize equipment such as Hoyer lifts for safe patient transfers when necessary.
- Engage clients in social activities to promote mental well-being and social interaction.
- Follow HIPAA guidelines to maintain confidentiality of client information.
- Collaborate with healthcare teams to develop and implement individualized care plans.
Experience
- Previous experience in caregiving or support roles within home health, hospice care, nursing homes, or long-term care settings is preferred.
- Familiarity with patient observation techniques and monitoring vital signs is essential.
- Knowledge of first aid and CPR certification is highly desirable.
- Experience working with individuals with autism or other developmental disabilities is a plus.
- Strong interpersonal skills with the ability to build rapport with clients and their families.
- Ability to perform heavy lifting and assist clients physically as needed.
