We are seeking a highly skilled and adaptable Licensed Practical Nurse (LPN) to join our team as an On-Call Casual Client Care Assessor & Planner.
This vital role focuses on the initial and ongoing assessment of clients in their homes, developing comprehensive, goal-directed Care Plans, and ensuring the safe and effective delegation of care tasks to our team of Home Care Aides (HCAs). This position requires excellent clinical judgement and critical thinking to manage the start of care and support care continuity.
Schedule & Availability
This is an On-Call Casual role. Availability is a key component, as you will be relied upon to fill shifts as needed, often to conduct new client assessments or urgent care plan reviews.
- Mandatory Requirement: Availability for at least one weekend (Saturday and/or Sunday) is required to meet the critical staffing and assessment needs of our clients.
- Typical Shift Structure (as needed): Shifts will primarily be scheduled during regular day hours to conduct assessments but may include coverage for:
- Morning/Day Assignments: For new client admissions and urgent reassessments.
- Scheduled Follow-ups: To evaluate and update established care plans.
Key Responsibilities
As an LPN Client Care Assessor & Planner, you will lead the clinical start-of-care process, ensuring every client receives a personalized and expertly planned care program.
Client Assessment & Care Planning
- Conduct comprehensive, holistic client assessments (physical, psychosocial, and environmental) in the client's home to determine their specific needs and goals.
- Formulate a nursing diagnosis and use clinical judgment to develop and implement an individualized Care Plan that is evidence-informed and aligns with ComforCare's client-centered approach.
- Lead client admissions, conduct transfers, and oversee discharge planning from a nursing perspective.
- Complete all necessary documentation, including working with MDS Assessments and utilizing electronic documentation systems (such as PointClickCare) to ensure records are accurate and up-to-date.
- Continually evaluate the effectiveness of the care plan and update it to reflect the client’s progress or changing health status.
- Obtain and manage physician's orders to ensure the care plan is current and medically sound.
- Coordination, Delegation & Collaboration
- Delegate care tasks to Caregivers, in accordance with BCCNM standards, ensuring the tasks are within the LPN's scope, the Caregivers's training, and the client's specific care plan.
- Provide necessary coaching and direction to care providers to ensure the delegated tasks are performed safely and competently.
- Facilitate resolutions to complex situations, applying strong critical thinking and decision-making skills.
- Participate in Care Conferences and liaise with physicians, families, and other members of the interdisciplinary team.
- Ensure smooth continuity of care by communicating clear instructions and client status updates across the care team.
Professional Practice & Standards
- Practice strictly within the scope, limits, and standards dictated by the British Columbia College of Nurses and Midwives (BCCNM) for LPNs.
- Ensure clients’ rights of privacy and confidentiality are maintained in all aspects of assessment and documentation.
- Demonstrate the ability to plan, organize, and prioritize assessment, planning, and documentation workload effectively.
Job Requirements
Professional & Technical
- Current practicing registration as a Licensed Practical Nurse (LPN) with the British Columbia College of Nurses and Midwives (BCCNM).
- Graduation from a recognized program for Licensed Practical Nurses.
- Recent, related experience as an LPN providing assessment and care planning in a healthcare environment. Previous experience in Home Care or Long-Term Care is preferred.
- Current First Aid & CPR certification.
- Valid Food Safe certification.
- Valid WHMIS certification.
Essential Conditions
- Valid BC Driver's License and access to a reliable vehicle (required for travel to client homes throughout the Vancouver West area).
- Completed Criminal Record Check.
- Covid-19 Immunization as per public health orders.
- Negative TB Test (Two-Step).
- Up-to-date Immunization record.
