We currently serve over 300 Skilled Nursing Facilities including sub-acute, long-term care, and assisted livings throughout NJ, NY, PA, and MD.

Key Responsibilities

• Conduct individual and group psychotherapy sessions for patients in a sub‑acute or skilled nursing facility.
• Assess, diagnose, and treat a wide range of emotional and behavioural health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
• Develop and implement evidence‑based treatment plans tailored to the needs of each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician‑designed electronic medical/health system (EHR).
• Monitor patient progress and adjust treatment plans as needed.

Requirements

• Active and unrestricted licence to practice as a Licensed Marriage and Family Therapist in the state applying for.
• Effective oral and written communication in English.
• Basic proficiency with technology, including electronic health records (EHR).
• Strong organisational and documentation skills, with attention to regulatory compliance.
• Ability to provide in‑person services at assigned facilities, up to 45 minutes.
• Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
• Full vaccination and booster status may be required in some facilities.

Preferred Qualifications

• Previous clinical experience in a sub‑acute, long‑term care, or geriatric healthcare setting.
• History of treating anxiety, depression and adjustment disorders.
• Excellent time‑management and organisational skills.

Psych or Addictions experience is a plus.
