Job summary

A community health organization in Guelph, Ontario is looking for a full-time Supportive Addiction & Mental Health Support Worker. This role involves direct case management to assist individuals with substance use and mental health issues, providing housing coordination, and supporting client access to various services. Qualifications

  • Minimum one year of progressive experience in addictions or mental health.
  • Completion of a College Diploma or University Degree relevant to mental health, addiction, or social services.
  • Valid G Driver’s License and Satisfactory Motor Vehicle Record.

Responsibilities

  • Conduct intake and enrollment activities, assisting clients with gathering eligibility documents.
  • Create and implement individualized care plans in collaboration with community partners.
  • Maintain appropriate confidentiality and documentation standards.

Skills

Experience in addictions or mental health
Case management knowledge
Experience in social service settings
Crisis intervention education
