Job Summary
The Hospital Pharmacist is responsible for providing expert pharmaceutical services to ensure the safe, clinically effective, and cost-efficient use of medicines for patients throughout the hospital. The pharmacist works as an integral part of the multidisciplinary healthcare team, participating in the delivery of pharmaceutical care on wards, in the dispensary, and in specialized clinical areas.
Responsibilities
- Clinical care: Monitor, review, and evaluate prescriptions for accuracy and therapeutic appropriateness, including drug interactions, dosages, and patient history.
- Clinical care: Participate in ward rounds and collaborate with medical and nursing staff to optimize medication therapy and ensure the best possible patient outcomes.
- Clinical care: Perform medication reconciliations upon patient admission, transfer, and discharge to prevent medication errors.
- Clinical care: Provide expert advice to healthcare professionals on drug selection, administration, and adverse drug reactions.
- Dispensing and medication management: Accurately dispense and check medications for hospital inpatients and outpatients, including sterile and non-sterile preparations.
- Dispensing and medication management: Supervise and support pharmacy technicians, pre-registration pharmacists, and other junior staff in dispensing and clinical tasks.
- Dispensing and medication management: Manage and monitor stock levels, ensuring the secure and appropriate storage of all medicines, including controlled drugs.
- Dispensing and medication management: Oversee the preparation and quality-checking of tailor-made medications for individual patients.
- Patient education and counseling: Counsel patients and their carers on how to properly use their medicines, including potential side effects and correct storage.

- Safety and governance: Contribute to the hospital's clinical governance and risk management agenda by reporting medication errors and near misses.
