POSITION: Support Worker 3

REGION: Supported Living - East

HOURS OF WORK: 37.5 hours per week

As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals with an intellectual disability, as well as support to their families, including residential and day support, assistance with employment, community support, early childhood services, and respite.

QUALIFICATIONS:

  • Post-secondary education with a focus on Social Services or Developmental Services is required, along with knowledge of healthcare procedures and mechanical equipment such as wheelchairs, lifts, G-tubes, and other specialized equipment.
  • Post-secondary education in a non-related field will require a minimum of 1 year of related work experience.
  • Successful completion of internal legislated and mandated training is required.
  • Valid driver’s license is an asset.

JOB SUMMARY:

Identifies the needs of individuals supported and delivers supports and programs consistent with their life plans to enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES:

  • Support the development of strategies for service delivery according to life plans and personal outcomes.
  • Provide case management to individuals on caseload and assist with case management of other individuals supported.
  • Ensure appropriate personal and physical care, crisis response, counseling, and emotional support.
  • Participate in assessing long-term service needs and identify opportunities to improve service delivery.
  • Coordinate and provide direct support in implementing life plans and monitor achievement of outcomes.
  • Maintain documentation, monitor finances, and ensure an appropriate standard of living for individuals supported.
  • Locate housing and employment opportunities for individuals supported.
  • Provide direction to staff regarding service delivery.
