Position Summary

The Client Care Supervisor plays a key role in coordinating and monitoring high‑intensity home care services. This role is responsible for assessing client needs, determining eligibility, developing individualized care plans, and ensuring clients receive the appropriate services to support safe and effective care at home.

You will work directly with clients, caregivers, and health care partners, ensuring timely access to services and providing ongoing leadership and oversight to support quality care delivery. Although you may be assigned to a primary location, flexibility to support other sites as needed is required.

Work Schedule: 40 hours/week (8-hour shifts) scheduled between 7:00 a.m. and 8:00 p.m.

Key Responsibilities
  • Conduct comprehensive assessments in collaboration with clients and families.
  • Determine eligibility for services and develop individualized care and service plans.
  • Coordinate referrals and link clients with appropriate service providers.
  • Monitor service delivery and adjust care plans as client needs evolve.
  • Establish positive and supportive relationships with clients, families, and caregivers.
  • Balance client needs with available resources while respecting client values and preferences.
  • Maintain accurate and timely documentation in all required systems and forms.
Qualifications
  • Registered Practical Nurse (RPN) or Master of Social Work (MSW) in good standing with the applicable regulatory body.
  • Minimum 1 year of recent community health, social work, or related experience.
  • Strong understanding of the health care system, home care services, and community resources.
  • Excellent communication, assessment, interpersonal, problem‑solving, and decision‑making skills.
  • Effective time management, organization, and the ability to work independently and collaboratively.
  • Demonstrated accuracy in completing documentation and reports.
  • A valid driver’s license and access to a reliable vehicle for home visits within York Region.
  • Proficient with Windows‑based computer systems.
  • Current CPR & First Aid certification.
  • Proof of a negative TB skin test (2‑step Mantoux).
  • Clear Vulnerable Sector Police Check (completed within the last 6 months).
Assets
  • Experience working with diverse populations (e.g., multicultural communities, palliative, mental health, geriatrics, and dementia care).
  • Case management or recent home/community care coordination experience.
  • Ability to speak French or additional languages.
