The Care Manager is responsible for achieving sustained growth and success through exceptional care delivery and strong support of our frontline team. This role works collaboratively with the Scheduling and Coordination teams to ensure service quality, reliability, and responsiveness across all clients and caregivers.

The ideal candidate is a hands-on, people-centered leader with a proven ability to scale operations without compromising quality. They build strong relationships with staff, clients, and community partners, driving both operational performance and an outstanding care experience.

Responsibilities:

Lead and support caregivers, ensuring they have the tools, guidance, and confidence to deliver exceptional care.

  • Oversee and mentor a small team of Care Consultants who support client assessments, reassessments, and ongoing care coordination.
  • Steward the new client onboarding and assessment process, ensuring smooth implementation of care plans and alignment with service standards.
  • Drive operational excellence, reliability, and responsiveness across assigned clients, caregivers and support teams.
  • Build & Maintain strong community and referral relationships to support sustainable growth and enhance Kindred’s reputation for service quality.
  • Maintain oversight of incident reporting and resolution within your region, exercising sound judgement to ensure timely and appropriate responses.
  • Work in a hybrid environment, combining remote work with frequent travel within your region to support caregivers, clients and community partners.
Qualifications:
  • Must be a Registered Nurse (RN) in good standing.
  • 5+ years of progressive leadership experience, including direct oversight of large, diverse teams.
  • Proven ability to lead within a scaling organization, balancing people leadership with operational execution.
  • Demonstrated success in driving client satisfaction, service quality, and retention outcomes in a community, home care, or healthcare setting.
  • Strong analytical and problem-solving skills with the ability to make sound, timely decisions.
  • People first leader with a coaching mindset and the ability to build trust and accountability across distributed teams.
  • Action-oriented and solutions-driven; takes ownership and executes effectively.
  • Must reside in Southern Western New Brunswick.
