Qualifications

• Charge Nurse experience as per DOH PQR with 2+ years in Case Management.
• Staff Nurse experience as per DOH PQR with 4+ years in Case Management.
• Current SEHA Senior Charge Nurse or completion of SEHA Case Management Training.

Responsibilities

• Coordinate care plans for patients to ensure quality care.
• Facilitate necessary resources and maintain documentation.
• Monitor patient progress and intervene when necessary.
