Job summary

Highmark Health is seeking a Wellness/Lifestyle Coach to oversee a defined panel of members, assess health management needs, and coordinate with a multidisciplinary team to improve outcomes.

You will engage members telephonically or virtually, develop personalized care plans, address barriers, and connect members with resources to achieve personal health goals. Documentation and quality indicators are integral to the role.

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness model/program delivery environment

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health
• Maintain oversight over specified panel of members by performing ongoing assessment of health management needs and triaging to resources
• Create care plans addressing identified needs with long- and short-term goals and regular contact for progress reassessment
• Demonstrate clinical outcomes and cost savings based on wellness interventions
• Ensure all activities are documented and compliant with requirements
• Participates in workgroups to improve wellness coaching efficiency and program development
• Other duties as assigned or requested
