Job summary

Highmark Health is seeking a Wellness/Lifestyle Coach to lead a panel of members with varying health needs. You will develop personalized care plans, coordinate with a multi-disciplinary team, and conduct outreach to improve health outcomes via telecommunication or virtual visits.

The role monitors quality outcomes and supports program pilots. The ideal candidate has a Master’s in Nutrition or related field and at least 3 years in wellness coaching within a health plan or disease management
