Job summary

Highmark Health is seeking a seasoned wellness coach to oversee a defined member panel with varying health needs, delivering personalized care plans and coordinating with a multidisciplinary team to improve health outcomes.

You will engage with members via telephone, digital, or virtual means, monitor progress, and document activities to ensure compliance and high-quality care across regions and programs.

Qualifications

• Master's degree or equivalent in a health-related field or demonstrable alternative education as approved by the company.
• 3 years of experience as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.

Responsibilities

• Provide care plans addressing identified member needs with short- and long-term goals and regular follow-up.
• Coordinate with multi-disciplinary resources to support member health management.
• Engage with members via telephonic, digital, or virtual channels to improve outcomes.
• Document activities in compliance with regulatory and organizational standards.
• Support development and implementation of new wellness programs and pilots.
