Job summary

Highmark Health is seeking a Wellness/Lifestyle Coach to lead a dedicated panel of members, assessing health management needs and delivering personalized care plans through telephonic, digital, or virtual outreach. You will collaborate with a multidisciplinary team to improve clinical, financial, and functional outcomes.

Qualifications

• Master's Degree as required; or relevant experience in lieu of degree.
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused environment

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee specified panel of members by assessing health management needs and triaging to appropriate resources.
• Create care plans with long- and short-term goals and regular contact for progress re-assessment.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure activities are documented and compliant with regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and member-centric wellness services.
• Other duties as assigned.
