Job summary

Highmark Health seeks a seasoned Wellness/Lifestyle Coach to manage a panel of members, assess health management needs, and develop personalized care plans. You will coordinate with a multidisciplinary team to refer members to appropriate resources and monitor health outcomes through telephonic, digital, and virtual outreach.

Responsibilities include applying evidence-based interventions, maintaining documentation for regulatory compliance, and contributing to program improvements.

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree.
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in wellness-focused environment.

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Create care plans for assigned members, remove barriers to care, identify resources, and plan regular contacts for re-assessment of progress towards goals.
• Demonstrate clinical outcomes and cost savings from wellness interventions and support members across all ages and regions.
• Ensure all activities are documented and compliant with requirements and standards.
