Job summary

Highmark Health seeks a Wellness/Lifestyle Coach to oversee a panel of members, assessing health management needs and coordinating with a multi-disciplinary team. The role emphasizes personalized care plans, goal setting, and regular contacts to improve health outcomes.

You will engage with members telephonically or digitally, ensuring high-quality interventions across nutrition, tobacco cessation, sleep, weight, and maternal health areas while maintaining compliance with regulatory standards.

Qualifications

• Master’s degree or equivalent experience as determined by the company
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused setting
• Preferred certifications: Tobacco Cessation, Registered Dietitian, CDE, Exercise Physiology, Lactation Counselor, NDPP Lifestyle Coach, CHWC

Responsibilities

• Maintain knowledge of evidence-based interventions to deliver high-quality lifestyle coaching across multiple areas
• Oversee a specified member panel and assess health management needs, triage to resources as needed
• Create care plans with long- and short-term goals and regular contact for progress re-assessment
• Demonstrate clinical outcomes and cost savings from wellness interventions across diverse populations
• Ensure documentation complies with all regulatory and accreditation standards
• Participate in workgroups to improve program efficiency and member-centric focus
• Perform other duties as assigned
