Job summary

Highmark Health is seeking a Wellness Coach to oversee a defined member panel, assess health management needs, and design personalized care plans with proactive outreach via telephone, digital, or virtual channels. The role focuses on delivering evidence-based lifestyle interventions and coordinating resources to improve health outcomes.

The incumbent collaborates with a multi-disciplinary team, tracks quality outcomes, and ensures compliance with regulatory standards while supporting program

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree
• 3 years as a Registered Dietician or Exercise Physiologist or Wellness/Lifestyle Coach in a wellness-focused program delivery environment

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards for lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee a specified panel of members, assess health management needs, and triage to appropriate resources.
• Create care plans with goals and regular contacts to reassess progress.
• Demonstrate clinical outcomes and cost savings from wellness interventions across diverse populations.
• Ensure activities comply with regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and member-centric wellness services.
• Perform other duties as assigned.
