Highmark Health is seeking a wellness coach to oversee a panel of members, assess health needs, and develop personalized care plans. You will collaborate with multidisciplinary teams to coordinate care and promote quality outcomes across populations.

The role emphasizes delivering lifestyle interventions, maintaining documentation to regulatory standards, and participating in program development and pilots to enhance wellness services.

Formación

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused environment

Responsabilidades

• Maintain knowledge of evidence-based interventions to deliver high quality lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee a specified panel of members and assess health management needs, triaging to resources as needed.
• Create care plans with long- and short-term goals and regular contact for re-assessment of progress.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Document activities in compliance with regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and member-centric wellness services.
