Highmark Health seeks a Wellness/Lifestyle Coach to oversee a panel of members with varied health needs. You will assess health management needs, develop personalized care plans, and coordinate resources to improve outcomes.

You will engage members through calls, digital channels, and virtual visits, tracking progress and maintaining compliant documentation. You will work with a multi-disciplinary team to demonstrate clinical outcomes and cost savings from wellness interventions, supporting

Qualifications

• Master's degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience considered in lieu of master’s degree.

Responsabilités

• Deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition and physical activity.
• Create care plans for assigned members, address barriers, identify resources, and set short- and long-term goals.
• Engage members via telephonic, digital, or virtual channels to improve health outcomes.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Document activities in compliance with applicable regulations and standards.
• Participate in workgroups to improve wellness coaching programs and pilots.
• Perform other duties as assigned.
