Job summary

Highmark Inc. seeks a qualified wellness coach to oversee a member panel, develop personalized care plans, and coordinate with a multidisciplinary team to improve health outcomes. The role involves telephonic and digital engagement, behavior-change coaching, and ongoing assessment of progress against goals.

You will apply evidence-based lifestyle interventions, document compliance, and contribute to program pilots, while supporting diverse populations across health services and products.

Qualifications

• Master's degree or equivalent in a relevant field.
• 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach.
• Experience in a wellness model or disease/condition management setting.

Responsibilities

• Maintain knowledge of evidence-based interventions for tobacco cessation, weight management, sleep, stress, nutrition, activity, and maternal health.
• Oversee a panel of members, assess health needs, and triage to appropriate resources.
• Create care plans with goals and regular contacts to reassess progress.
• Demonstrate outcomes and cost savings from wellness interventions.
• Ensure all activities are documented per regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and member focus.
• Perform other duties as assigned.
