CVS Health is seeking a Care Management Associate to coordinate medical services, support care plans, and promote effective utilization of health services. You will triage Care Team tasks, screen patients, and make referrals to ensure timely and appropriate care.

The role requires strong communication, ability to work across clinical teams, and comfort with multiple systems. Weekend/holiday hours are expected, with a structured training period and 40-hour weeks.

Qualifications

• 2-4 years in healthcare settings or medical billing/coding.
• Experience in a call center or customer service environment.
• Ability to manage multiple systems and meet daily targets.
• Proficient with Microsoft Office 365 and clinical platforms.
• Excellent written and verbal communication skills.

Responsabilités

• Review and triage Care Team tasks and referrals.
• Identify admission reasons and apply intervention tools.
• Screen patients and coordinate required medical services.
• Refer to medical staff and monitor care plan implementation.
• Coordinate care delivery with nurses and providers.
• Support documentation and data entry in health systems.
• Assist in claims resolution and regulatory compliance.
