The role requires 2–4 years in healthcare, strong communication, and proficiency with Microsoft Office 365 and clinical platforms. A high school diploma or GED is required, with weekend hours and full-time hours in a remote home-based setting within your state.

Qualifications

• 2-4 years of experience in healthcare settings such as medical/health services.
• Experience in a call center or customer service environment.
• Ability to work independently and meet deadlines with an outgoing, caring presence telephonically.
• Strong written and verbal communication skills.
• Ability to navigate multiple computer systems and digital tools.

Responsibilities

• Perform initial review and triage of Care Team tasks.
• Identify reasons for admission and apply intervention assessment tools.
• Screen patients to identify needed medical services.
• Coordinate services and refer to appropriate care teams.
• Support development and implementation of care plans.
• Coordinate health care service delivery with nurses or physicians.
