Job summary

CVS Health is seeking a Weekend Care Management Associate to support care coordination, intake, screening, and implementation of care plans. You will liaise with nurses and medical staff to ensure appropriate services while promoting efficient health care utilization.

The role requires 2–4 years in healthcare, strong communication, and ability to work across multiple systems. Training runs 4–8 weeks, with weekend 10-hour shifts and local time zone coordination.

Qualifications

• 2–4 years of experience in healthcare, medical/health setting, medical billing or coding.
• Experience in call center or customer service environment.
• Ability to work independently and meet deadlines with an outgoing, caring presence.
• Strong written and oral communication skills.
• Ability to work across multiple systems including Office 365, clinical platforms and health plan systems.
• Technically proficient and able to navigate computer systems and digital tools.
• Ability to collaborate with clinical teams.
• Excellent communication with providers and internal stakeholders.

Responsibilities

• Review and triage Care Team tasks and referrals.
• Identify reasons for admission, facility, and member products for intervention assessment.
• Screen patients to identify needed medical services.
• Refer to medical staff and coordinate services per benefit plan.
• Monitor non-targeted cases for discharge date and disposition.
• Coordinate care plan development and implementation.
• Use Med Compass and Aetna systems to manage member information.
• Promote internal and external communication to enhance medical management.
