Job summary

CVS Health is seeking a Care Management Associate to support coordination of medical services, triage of Care Team tasks, and development of care plans under nurse direction to promote efficient health care use.

You will coordinate referrals, monitor cases, and communicate with providers and teams using Med Compass and EHR portals while maintaining HIPAA compliance. Training hours run 8:00 am–7:00 pm EST, with full-time schedule and 40 hours weekly.

Qualifications

• 2-4 years of experience in healthcare or medical setting.
• Experience in call center or customer service.
• Ability to work independently and meet deadlines with strong written and oral communication.
• Proficient with Microsoft Office 365 and clinical systems.

Responsibilities

• Initial review and triage of Care Team tasks.
• Coordinate and implement care plans under direction of nurse/medical director.
• Identify service needs, refer to appropriate medical services, and monitor outcomes.
• Communicate with providers and internal teams to improve care delivery.
