Weekend 10-hour days, with training, and a focus on accurate, timely care delivery. This remote/full-time role emphasizes teamwork with care teams, adherence to standards, and providing high-quality care

Qualifications

• 2-4 years of experience in healthcare or medical setting.
• Experience in call center or customer service environment.
• Ability to work independently and meet deadlines with a caring demeanor.
• Strong written and oral communication skills and ability to work across multiple systems.

Responsabilités

• Initial review and triage of Care Teams tasks.
• Identify reasons for admission and match interventions.
• Screen patients and refer to medical services as needed.
• Coordinate care plan implementation under clinical direction.
• Communicate with providers and care team to optimize care.
