CVS Health is seeking a Care Management Associate to coordinate care plans and support the care team from a remote, full-time role. You will screen and triage cases, identify services needed, and apply clinical workflows to ensure efficient care delivery.

You will work from home up to 40 hours per week, using Microsoft Office 365 and healthcare systems to coordinate with providers while maintaining HIPAA compliance. Strong communication and organization are essential.

Qualifications

• 2-4 years of experience in healthcare field, medical billing or coding preferred.
• Experience in a call center or customer service environment.
• Ability to work independently and meet daily deliverables with a caring, professional presence telephonically.
• Strong written and oral communication skills with attention to detail.
• Proficiency navigating multiple systems and clinical platforms.

Responsabilités

• Support comprehensive coordination of medical services and care team tasks.
• Triage and initial review of care team referrals and intervention needs.
• Coordinate care plan development and care delivery within the benefit plan.
• Promote effective utilization of healthcare services and provider communication.
