CVS Health is seeking a Weekend Care Management Associate to support comprehensive coordination of medical services, triage Care Team tasks, and implement care plans. The role emphasizes HIPAA-compliant handling of member information, cross-team collaboration, and efficient use of health plan systems.

Responsibilities include identifying service needs, coordinating referrals, and ensuring timely care delivery while working within 8:00 AM–8:00 PM ET training shifts and weekend hours.

Qualifications

• 2-4 years of experience in healthcare settings or medical/health services.
• Experience in call center or customer service environments.
• Ability to work independently, meet deadlines, and maintain a caring presence telephonically.
• Strong written and oral communication; able to meet daily metrics with speed and accuracy.

Responsibilities

• Perform initial review and triage of Care Team tasks.
• Identify reasons for admission and apply intervention assessment tools.
• Screen patients to identify needed medical services and coordinate referrals.
• Support development and implementation of care plans under nurse/clinical direction.
• Coordinate health care delivery in the most appropriate setting and cost.
• Promote communication with providers and care team members to improve management.
