Job summary

CVS Health is seeking a Care Management Associate to support coordination of medical services, triage tasks for the Care Team, and promote efficient use of healthcare resources. You will collaborate with nurses, clinicians, and health plan staff to ensure timely, quality patient care.

Training occurs during onboarding with flexible daily scheduling from 8:00 am to 7:00 pm EST. This role supports remote work from a designated home location and requires proficiency in Microsoft 365, EHR systems,

Qualifications

• 2–4 years in healthcare or medical setting.
• Experience in call center or customer service environment.
• Able to work independently while meeting daily metrics.
• Strong written and verbal communication skills.

Responsibilities

• Support comprehensive coordination of medical services and care plan implementation.
• Triage Care Team tasks and identify intervention needs.
• Coordinate and refer for required services with providers and health plan teams.
• Maintain HIPAA-compliant, accurate documentation and case records.
