Job summary

Mercy is seeking a Nurse Manager for the Medical Management & Transition Team to lead virtual-based transitional care programs, including ED-to-Home, chronic condition follow-up, and intensive care coordination. You will oversee daily operations, quality outcomes, staff performance, and regulatory compliance while driving population health initiatives.

The role requires a nursing degree, current licensure, and several years of clinical leadership experience.

Qualifications

• Graduate of an approved school of nursing.
• Current licensure by the State Board of Nursing in applicable state.
• Four years of clinical experience with management/supervision elements.
• Certifications: CPR and NRP.
• Medication safety and math competency testing for direct patient care.
• BSN or higher degree preferred; MS/MPH or similar welcomed.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual transitional care programs.
• Ensure safe, effective, patient-centered care delivery and regulatory compliance.
