Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, overseeing virtual-based transitional care programs, including ED-to-Home acceleration and chronic condition follow-up.

You will drive safe, patient-centered care, foster interdisciplinary collaboration, and advance population health strategies while ensuring quality outcomes and regulatory compliance through strong operations and staff performance management.

Qualifications

• Graduate of an approved school of nursing; BSN or completing BSN within 2 years.
• Current RN licensure in the applicable state.
• Four years of clinical experience with supervisory/management experience preferred.
• CPR and NRP certifications.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual transitional care programs.
• Oversee daily operations, quality outcomes, and staff performance.
• Ensure regulatory compliance across services and care transitions.
• Foster interdisciplinary collaboration and patient-centered care.
• Advance population health strategies through technology and care coordination.
