Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, overseeing virtual-based transitional care programs and population health initiatives. You will guide daily operations, ensure safe, effective, patient-centered care, supervise staff, and promote cross-functional collaboration to improve outcomes and regulatory compliance.

This role emphasizes strategic leadership, performance improvement, and a commitment to Mercy's mission in modern, technology-enabled care

Qualifications

• Graduate of an approved nursing program.
• Current licensure by the State Board of Nursing.
• 4 years of experience in clinical specialty or related area.
• CPR and NRP certifications.
• Medication and math competency testing required before medication administration responsibilities.
• BSN or MS preferred.
• 2+ years supervisory or management experience preferred.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual care programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Advance population health strategies through population health initiatives.
• Foster interdisciplinary collaboration and ensure timely follow-up and care coordination.
• Support ED-to-Home and intensive care coordination initiatives.
