Job summary

Mercy is seeking a Nurse Manager for the Medical Management & Transition Team to lead virtual-based transitional care programs. You will guide operations, ensure quality outcomes, supervise staff, and maintain regulatory compliance while coordinating care across the ED-to-Home and chronic condition follow-up programs.

The role emphasizes safe, patient-centered care, collaboration across disciplines, and ongoing improvement in population health through technology-enabled care.

Qualifications

• Graduate of an approved school of nursing.
• Current RN licensure in the applicable state of practice.
• 4+ years of clinical experience with supervision/leadership responsibilities.
• CPR and NRP certifications.
• Medication and math competency test for licensed staff before medication administration.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual transitional care programs.
• Ensure safe, effective, patient-centered care delivery and regulatory compliance.
• Oversee daily operations, quality outcomes, staff performance and interdisciplinary collaboration.
• Advance population health strategies and ensure timely follow-up and care coordination.
