Job summary

Mercy seeks a Nurse Manager for the Medical Management & Transition Team to lead virtual-based transitional care programs. You will direct daily operations, ensure high-quality patient-centered care, and drive performance improvement across ED-to-Home and chronic condition follow-up initiatives.

You will work to advance Mercy’s mission through innovative care management, technology enablement, and collaboration with interdisciplinary teams.

Qualifications

• Graduate of an approved school of nursing.
• Current licensure by the State Board of Nursing.
• Four (4) years of experience in a clinical specialty or related area.
• Certifications: CPR NRP.
• Must pass a medication and math competency test before direct patient care.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Foster interdisciplinary collaboration and ensure timely follow-up and care coordination.
• Advance Mercy’s mission through population health strategies and high-quality virtual care management.
