Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, driving operational excellence and strategic direction for virtual-based transitional care programs. The role focuses on safe, effective care delivery, quality outcomes, and regulatory compliance across the care continuum.

You will oversee ED-to-Home and intensive care coordination initiatives, promote interdisciplinary collaboration, and ensure timely follow-up while advancing Mercy’s population health strategies.

Qualifications

• BSN or higher anticipated within 2 years.
• Current RN licensure by the State Board of Nursing.
• 4+ years of experience in clinical specialty or related area; includes supervision, performance improvement, budgeting.
• CPR and NRP certifications.
• All licensed clinicians must pass a medication and math competency test before handling meds.
• Preferred: BS/MS; preferred: 2+ years supervisory/management experience.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care and related programs.
• Ensure safe, effective, patient-centered care; foster interdisciplinary collaboration.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Lead programs including ED-to-Home, accelerated outpatient management, and intensive care coordination.
• Mentor staff and manage budget while ensuring timely follow-up and care coordination.
