Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, overseeing virtual-based transitional care medical management and related wellness programs. You will direct daily operations, ensure quality outcomes, supervise staff, and maintain regulatory compliance across ED-to-Home and chronic condition follow-up initiatives.

You will promote safe, patient-centered care, foster interdisciplinary collaboration, and advance Mercy’s population health strategies through

Qualifications

• Education: Graduate of an approved school of nursing; BSN or in progress within 2 years.
• Licensure: Current licensure by the State Board of Nursing in the applicable state.
• Experience: Four (4) years in clinical specialty or related area with supervision, teaching, and budget.
• Certifications: CPR and NRP.
• Other: Medication and math competency test required for direct patient care staff.
• Preferred Education: BS/MS preferred.
• Preferred Experience: At least 2 years supervisory or management experience preferred.
