Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to lead accurate MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual Regulations. The role requires an active RN license, 2 years of nursing experience (geriatrics preferred), and CRN C certification. CPR is required by hire and must remain current.

You will ensure timely submissions, educate staff on regulatory updates, and collaborate on interdisciplinary care planning while upholding Life Care standards.

Qualifications

• Associate or bachelor’s degree in nursing and an active RN license.
• CRN C certification (clinical compliance) required.
• CPR certification by hire date and maintained during employment.
• Minimum 2 years of nursing experience; geriatric experience preferred.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by the MDS to the DON.
• Educate direct care staff about CMS RAI updates affecting documentation.
• Assist with review of Interdisciplinary Comprehensive Care Plan and Final Validation Reports.
• Attend PPS and other meetings; support compliance in documentation and billing processes.
• Perform staff nurse functions as required and maintain patient safety and rights.
