Job summary

Life Care Centers of America seeks an RN MDS Coordinator to coordinate and assist with the accurate and timely completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans in alignment with CMS RAI Manual Regulations and Life Care standards.

In this role, you will work with the nursing team to ensure documentation accuracy, provide education on CMS updates, and support the review of comprehensive care plans across the facility.

Qualifications

• Associate’s or bachelor’s degree in nursing from an accredited college or university.
• Active RN license in good standing in the applicable state.
• Two years’ nursing experience; geriatric nursing preferred.
• CPR certification upon hire or during orientation and must remain current.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by MDS to DON.
• Provide education to direct care staff regarding CMS updates or regulations affecting documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and attest that assessments are complete and accepted into CMS QIES before billing.
• Review CMS Reports to identify late submissions and develop systems to prevent recurrence.
• Attend PPS meetings and other required meetings.
• Perform functions of a staff nurse as required.
• Demonstrate dependable attendance and professional conduct.
