Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to coordinate and assist with the completion and submission of accurate MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI manuals and Life Care standards.

The role requires an RN with a nursing degree and active license, plus CPR certification. Geriatric experience is preferred, and ongoing education on CMS updates is expected to support compliant documentation.

Qualifications

• Requires nursing degree and active RN license
• Two years of nursing experience, geriatric experience preferred
• CPR certification required at hire or during orientation

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS assessments, CCAs, and care plans per CMS RAI manual regulations
• Report changes in patient condition identified by MDS assessments to the DON
• Educate direct care staff on CMS RAI manual updates impacting documentation
• Review interdisciplinary care plans and Final Validation Reports for CMS QIES submission
• Ensure timely billing once all assessments are in CMS QIES system
