Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to coordinate and assist with the completion and submission of accurate MDS Assessments, CCAs, and Care Plans in line with CMS RAI guidelines.

The role requires an RN license, an associate or bachelor’s degree in nursing, and at least two years of nursing experience (geriatric experience preferred). CPR certification is required, with ongoing compliance and professionalism expected.

Qualifications

• RN license in good standing
• 2 years of nursing experience (geriatric preferred)
• CPR certification required
• Knowledge of CMS RAI and post-acute care regulations

Responsibilities

• Coordinate and assist with MDS Assessments, CCAs, and Care Plans per CMS RAI
• Report changes in patient condition to the Director of Nursing
• Educate direct care staff on CMS RAI updates
• Review Interdisciplinary Care Plans and CMS Reports
• Assist with billing readiness and CMS QIES submissions
• Attend PPS and other meetings and perform staff nurse duties as needed
• Maintain professionalism and patient rights and safety
• Maintain confidentiality and follow company policies
